MatchCares is a Community First Services and Supports (CFSS) and Personal Care Attendant (PCA) agency licensed by the Minnesota Department of Human Services. We serve individuals with disabilities, seniors, and families across all Minnesota counties.
We believe that great care starts with great matching. Our team works closely with clients, families, and case managers to understand each person's unique needs — then carefully pairs them with a caregiver who fits their life.

MatchCares provides in-home care services under Minnesota's Medicaid programs.
Hands-on assistance with activities of daily living (ADLs) including bathing, dressing, grooming, mobility, and eating for individuals who need physical support.
A flexible, person-centered model that allows individuals with disabilities to direct their own care — including selecting, training, and managing their caregiver.
Eligible family members can become paid caregivers for their loved ones under the PCA or CFSS program. We handle all paperwork, training, and payroll.
We work directly with case managers and social workers to coordinate care plans, maintain DHS compliance, and provide timely documentation and reporting.
